Final Project: Family Plan Assignment Instructions

Collaboration and Communication Family Plan

You will select an adult family member of someone with special needs to inquire about their school experiences specific to collaboration as it pertains to school supports and services.

List each question and then give the response underneath the question in a double-spaced typed format. Responses must be rich and detailed. It is appropriate to use first-person language throughout all 3 parts of this assignment.

Part 1: Demographics

Describe the relationship between the family member and the individual with the disability. In each interview, you will develop 8-10 interview questions that are applicable to strategies that help college students manage their time, their life, and their studies. Include the questions and a summary of the responses from each interview.

Part 2: Needs Assessment

1.     Describe how the family member perceives collaboration with the school.

2.     Explain the family’s expectations for their child.

3.     What collaboration strategies were effective in supporting the family and student?

4.     What suggestions did the family member give in terms of improving collaboration?

Part 3: Research Strategies Plan

You will research strategies to develop 3-5 realistic goals that could be established to ensure positive collaboration and communication among team members to support your findings in Part 2: Needs Assessment.

Incorporating the information gained through research and the family member interview, you will create a plan to meet the needs of college students. This assignment requires citations from 5 sources in current APA formatting. You must integrate your findings in Part 3 of the Final Project.

Note: The total page length for this assignment is 9-12 pages when you combine the Family Member Interview (3-4 pages), Needs Assessment (2-3 pages) and Research Strategies Plan (4-5 pages).
